PCHS GOALS AND OBJECTIVES

Ongoing Community Engagement and Assessment

  • Ongoing growth and expansion of PCHS Manitoba based on specific client and community needs.

 

Collaborative Approach

  • Health integration (mitigate barriers) through collaborative cultural support services in a manner which enhances outcomes and demonstrate value for money.

 

Culturally Inclusive

  • Culturally relevant care and support in a manner that respects the autonomy of South Asian clients and families.

 

Innovative

  • Establishing the appropriate Governance Model and Organizational Structure to facilitate health transformation.

 

 

Community Development and Capacity Building 

  • Planning, management and delivery of programs and services, inclusive of education, training and capacity building benchmarks.

 

 

This PCHS Manitoba Health Transformation Roadmap also includes very specific goals, objectives and measurable outcomes that will be achieved as a result of our strategic phased approach, including but not limited to the following:

 

  1.  Stakeholder engagement
  2. Education, training and capacity building (inclusive of research component)
  3.  Focus on community and regional needs
  4. Measurable health outcomes (increased level of community- based and culturally relevant and appropriate services and support)
  5. Capacity building and the provision of career paths for individuals and volunteers – to support existing service providers hence providing for a better quality of community-based care for patients and ensuring long-term success and sustainability.
