Health Transformation Roadmap

Through meaningful and ongoing community and stakeholder engagement, and through rigorous planning and focus, our goal is to accomplish the following:

Phase One: To provide patient advocacy, system navigation and cultural sensitivity support to families in crisis, inclusive of outreach services that meet the social and cultural needs of our diverse patient populations. 

Phase Two: To secure funding and resources to address the health services gaps and disparities and to ensure an equitable and patient centric level of care and support is available to our clients and their families. 

Phase Three: To become a recognized and accredited health and wellness service provider through a collaborative and sustainable approach with key public and private sector stakeholder partners to provide health, wellness, settlement and social services to diverse communities across Manitoba in a manner that supports kids and youth, families and seniors and undertake community development projects that strengthen our neighborhoods and our community. 

A comprehensive Environmental Scan was conducted between September and December of 2022, the results of which have now been reviewed and evaluated and that Environmental Scan is the basis of the PCHS Manitoba Health Transformation Roadmap and the phased approach which is highlighted in this first Strategic Plan
