Will facilitate health transformation and become the foundation for the legal framework agreement for funding that will be negotiated between key stakeholder partners and PCHS Manitoba. We are proposing a phased approach, which will ensure success and sustainability.
Consistent with the mission and objectives of PCHS Manitoba, this health transformation initiative seeks to improve the health and wellbeing of all south Asian communities and the region through the integration and enhancement of community-based health programs and services; that are reflective of the needs and priorities of the South Asian community through timely and culturally appropriate communication and support.
To ensure success and sustainability, the integration of appropriately funded and planned, education, training and capacity building initiatives are also an essential component of this health transformation process.
Community Planned:
- Self-planned, managed and delivered in a manner that is reflective of the needs and priorities of south Asian and diverse underserved communities across Manitoba.
- Broad prospective that acknowledges not only the health and wellness challenges in each community but also the social and cultural challenges and barriers
Adequately Resourced Health and Wellness Services
- More culturally relevant community-based care closer to home
- Fully integrated model which eliminates systems navigation and cultural barriers and provides timely and responsive access to care
- Increased access to health and wellness services that better aligns with accepted standards of care as regulated and prescribed by regulatory authorities and Canadian Institute for Health Information
Efficient Delivery Model with a Focus on Prevention, Wellness and Sustainability
- Care plans based on needs of patients and families with a care-teams approach
- Proactive health and wellness approach, leveraging research and data as opposed to a reactive approach, which results in fragmented care and long-term management of chronic conditions
Communication and Timely Dissemination of Information
- To all key stakeholder partners internal and external public sector and private sector
- Continue to increase community participation in health care delivery through education, training, and capacity building opportunities, that result in meaningful employment opportunities which will allow community-based health and wellness champions to emerge
- Ongoing community and stakeholder consultation with an annual review of the strategic direction and plan to ensure it remains aligned with goals and objective of PCHS Manitoba and key stakeholder partners.
Promote and Foster Ongoing Enhancement and Innovation
- Program Planning and Design, inclusive of education, training, and research
- Address fragmentation through a fully integrated model of care that reduces or eliminates operational and jurisdictional issues and enhances access to timely care and services for patients and families
